Registration Information


DMHA has moved to online Registration

 

Please read the outlines prior to clicking the online registration tab.

 

Registration is due by June 30th along with a 50% deposit fee to hold a spot on the team.

Registrations coming in after June 30th will have a $200 late fee added to them.

Final registration fees are due Oct 1st.

Payment will be accepted via cash, cheque, debit or credit. If paying debit or credit please contact our treasurer.

* For anyone in need of financial support options to play, please look into the Kidsport grant. 

 

Registration Fees include:

-Insurance & Registration with AAHA

-Ice Rentals (practice and home game ice)

-1 pair of hockey socks

 

Registration fee by age division:

Learn to Play       $182

U7                       $247

U9                       $450

U11                     $518

U13                     $571

U15                     $667

U18                     $731

 

Kitchen fees are $225 / player, and the AGM meeting fee are $50 / family.

If you are not working your kitchen hours you can date your check for Oct 1st.

If you are working your hours it will be post dated for the end of March.

AGM Meeting cheques are also post dated for the end of March. 

If fees have not been submitted your child will not be permitted on the ice.

 

*These above fees do not include AG Passes.

AG Passes are $40 for an individual or $80 for a family pass.

Each family may be required to help support the AG Society Fundraiser

 

 **It is mandatory for one parent/guardian from each family to attend the Annual General Meeting. Your $50 AGM cheque will be cashed if a parent is not present.

 

Players must be linked to a Respect in Sport profile prior to being able to register with Hockey Canada. 

This can be done by creating a profile on the Respect In Sport web page and completing the online course.

If you have completed the Respect and Sport course and it is still active but you have a new player to hockey, you must add the new player to your Respect In Sport profile.

Medical forms must be completed and given to the team manager and or coach. You can print a copy and bring it at the beginning of the season. Medical Form
